Intermittent Not in service on External hotline out

I have a BCM50 v205 with a PRI with 12 loops enabled all set to the default BlocB line pool. One of the phones (T7316e) is programmed with BlocB line pool access, but very infrequently on dial out he gets a "Not in Service" message on the set display. He is also set for pre-dial and a hotline to external using the routing table with external number 9. I also have second dial tone turned on for the PRI as well.
He is the only user that actively uses pre-dial and also seems to be the only one that gets the not in service messages. I have checked that only the 12 loops are provisioned in the PRI setup. I cannot change the non-provisioned loops to another line pool as it changes all lines on the PRI - so I assume the provisioning blocks them from being selected.
Ultimately I am stumped at why this intermittent not in service comes from - he only has access to the BlocB pool, so have I missed something in the setup?
Any help would be most appreciated!

You cannot set a user with hotline to automatically dial a 9 before they dial a number on a PRI circuit. The customer can use pre-dial, but they MUST include whatever is required by the destination code in order to access PRI, in this case a 9..
