Insert OLE Object displays icon but no graphic

I am running Crystal 9 on my PC and connecting to a SQL Server for my data. There is a form that has been created in Illustrator for a new promotion. The form is stored on my network currently, but I can move it if need be.

I want to Link or Embed this form (call it "form.ai") into my report with the intention of placing fields in the appropriate places over top of the form graphic.

Each attempt to link to or embed the form results in an icon displayed on the page. Printing the page only prints the icon. If I double-click the icon in my report, it opens the Illustrator file for editing.

Crystal Help files suggest that I should be seeing my graphic, especially if I embed it. What am I doing wrong?
 
I think I've found the answer to my own question:


It seems that either Illustrator 10 is not OLE-Automation enabled, or my file mapping is dropping the ball somewhere. Unfortunately, converting the form to another format loses text print quality.

I'm not sure what else to do, so I'll be re-creating this form in Crystal. How painful to have a three-click solution in front of you that doesn't want to work. Ah well :)
