Insert Invoice Recommendation for Payment

We manage construction projects as owner's representatives. One client, a hotel chain, asks us to review each submitted invoice, then stamp the invoice with a pre-designed form and enter the variable information into each field, such as the Vendor name, invoice amount, date, etc.

We would like to avoid a physical stamp and handwriting, and create some sort of interactive form in either Acrobat 6.0 Professional, Word, Excel, or Access. We'd fill out the form, and insert it somewhere on the invoice document emailed to us.

Can anyone help?

Thank you for your response. I'm not sure what information you need. The invoices themselves will either be received via email from the contractor as a .pdf file or will be scanned at our office to a .pdf file.

The current stamp is approximately 4" x 2", although we wouldn't need to duplicate its exact look as long as we capture the information. It looks something like:

PROJECT MGR'S INVOICE RECOMMENDATION FOR PAYMENT

Date: ______________________ Vendor: __________________
Hotel/Project: _____________ Service Type: _____________
Contract Type: _____________ Value ($): ________________
---------------------------------------------------------
Invoice recommended for payment by:
Project Mgmt Firm: ______________________________________
Invoice [ ] Pay App [ ] Approved Amt: $ ___________
____________________________ ___________________________
Signature Print Name

The Project Mgmt Firm, signature, and Print Name fields would be static for a given project manager, others would vary with each processed invoice.

We'd like to fill the information, perhaps from some other data entry form, and place the graphic into the .pdf file, preferably with the ability to reduce the contrast such that writing underneath can still be read.
