Index does not match the table

I am getting this message on a grid refresh. Error 114 - "Index does not match the table. Delete the index file and recreate the index." I have done this and I still get the error. How does this damage to the index file keep happening? How can I fix the table?
 
Indexes aren't normally that fragile. Are you really recreating the index or just doing a REINDEX? I'd be suspicious of the drive where this file exists. When was the last time a CHKDSK or DEFRAG was run on this drive? Is it on a server (Novell, NT, W2K), peer-to-peer (Win9x, NT), or is it Local?

If you are using a .cdx file, you may have to recreate all the tags associated with it.
 
I deleted every index associated with this table which made the .cdx file go away. Then I recreated each index. The table is on a Novell network drive.
 
Does it work anytime? That is, does it work at least once after the re-creation of the indexes and then fails eventually. Or does it simply not work ever? Is it possible that you are "fixing" one file, and the program is using another file? Is there any other application / user updating this file? (I've seen users open up a table in other programs - e.g. Excel - change the data and it doesn't update the index.)
