I have just installed InDesign CS5 and I have no Application Bar. I have searched through help files and can find no way of rectifying this.
Could anyone help?

Have you first uninstalled QuarkXPress? What version of QuarkXPress are you running?

It's ok after all. The book I was refering to was incorrect - it showed the application bar and menu bar as seperate. With CS5 they are together.
