Incoming calls sent immediately to General Delivery Mailbox

Have a BCM400 with an intermittent problem. Incoming calls are sometimes directed into the general delivery mailbox. Some of the lines this occurs on aren't even set to be answered by call pilot.
 
Do you have a ringing service configured? My first guess would be that you have a ringing service that is directing those lines to a set that doesn't have a mailbox built for it, and it's going to general delivery.
 
Not sure what you mean by "ringing service" but we do have some sets configured to ring.

What I forgot to mention in my first post was that the outside caller would hear just one ring and then the call would go into the general delivery mailbox. I started looking at my active DNs and noticed that a bunch of them were set to forward calls to x439. Don't know where that came from. We have neither a DN or mailbox with that number.
 
If set doesn't have a Vmail box then call goes to General Del.
