incoming calls disappear and hold not working

Hey everybody. This is the strangest issue i have ever seen! I have a 406V2 R5.0.8 with a PRI. I have one extension (4424 phone with a dss console) that randomly has issues anwsering calls. If you call it direct from outside (DID or main number via auto attendant) the phone will ring normally, but when you pick it up sometimes it will continue to ring for about 3 seconds and then the call will disappear! if you dont hang up and wait for another couple seconds, the call will come back and you are connected without pressing anything. Also sometimes if you try to place a recieved call on hold, the hold or park button does nothing. You can hit it mutiple times and it doesn't work. My first thought was a bad telephone or mabe even a port on the system, but i have replaced the phone, cables, crossconnects, dss console and power supply, and even moved the user to a new port on the sytstem. the problem still happens, so I deleted the extension completly and gave the phone a new user from scratch and a different extension number. same problem! The system status is not showing the call dropped when if disappears for that couple of seconds, it shows it connected to the user. I though mabe this maybe a pri issue, so i had the provider look at it, and they saw nothing (system status producing no errors or slips either)And this problem only happens to this one phone. No one else has a issue. I had this same problem before release 5.0.8 so i know if not a release bug. Has anybody had a similar problem. I dont know what more to do! this is crazy. mabe I can move her to a new desk on the roof and mabe the problem will fix itself!
 
From the docs, can you check it.

Add-on for 4412D+ and 4424D+ phones. Provides an additional 60 programmable buttons with a single lamp - red except for the bottom two rows which are green. Due to the single lamp not recommended for appearance functions as not all button states can be indicated. Up to two 4450 units can be connected to an existing phone. A maximum of 2 unit as supported per DS module or control unit. A maximum of 8 units total are supported on an IP Office system.

i think bas is saying, do you have more than one any one phone, or more than two on any one module/controller or more than 8 in total?

The system only has 2 dss 4450 consoles, one connected to seperate 4424 phones. the other 4424 with the 4450 works fine.
 
both dss consoles only have user buttons programmed on them. No appearance buttons or feature buttons. I am having the customer unplug the dss for the day tomorrow and see what happens( they dont use the dss very much.)
