Inbound Calls To Specific Extension Sounds Like an Obnoxious Fax Tone

We have a user who is intermittently experiencing a loud fax tone and sometimes what sounds like road noise from a cell phone on his inbound external calls only. The caller does not hear this and thinks no one has answered so they hang up but we hear the sounds on our end. The issue is noticed by his secretaries also (on his line only). So it sounds like an issue with the provider but they are saying it's not them (big surprise). They say they are seeing multiple invites on the failed calls. Our PBX vendor is not seeing this but still reviewing captures. We have CM6.2/Session Mngr 6.2 and SBC 6.2. We have SIP trunks for inbound/outbound. Calls go across the WAN to remote offices. We have a lot of offices but this is the only person having the problem. Phones are H323/9461G. I even logged him into another office and it still occurred. Just last week he logged his phone in at another location - back in his location Monday and this started. We also tried removing his setup from CM and re-adding it and swapped his phone also. Thoughts?
 
Does this only happen when someone dials there DID number?
Can you independently reproduce the issue by dialing there DID?
Have you looked at the SIP trace in the SBC or Session manager?
If you can not reproduce, then is this only when one person calls in?
 
Only to their DID; can be reproduced to their DID; our PBX vendor ran traces in all of our equipment - do not see anything; coming from multiple callers - different parts of the country - different clients. Provider has a PCAP - heard the noise and is asking for more PCAPs...they claim now it is not their problem but their PCAP this morning noted it happening before the call hit our equipment.
