i2002 phones - disconnecting incoming calls

I have an ITG 3.0 line side card with i2002 phone sets. I have had a rash of sets going bad, when the user goes to answer their phone with the line key or handsfree the set will drop the call.

Has anyone heard of this problem before?

doesn't sound like a phone, either a conjested network or a bad card. what does your qos look like
