html e-mail w/ form

It's easy to write up a basic HTML-mode e-mail in .NET. But inside that html e-mail, is there a way that I can include a form for the recipient to fill out (radio check this & textbox that) and reply/forward back that filled-out form to another e-mail?? It's a nice feature to have.. at least for my business.

Write it just as you would an html form with post and submit

I did that. The display looks fine, but the reply does not take the additional form radios/checks/textbox fields.
