It looks to me like a problem similar to an invalid cluster error in DOS/Windows. My first suggestion would be to run a repair on the filesystem, then restore the effected file from backup.
You might check the /usr/spool/mail files, and the /usr/adm/messages and /usr/adm/logs files for the error. Typically I would expect major failure messages to show up on the console, so if this didn't show on the console it may be related to an application the user was running, or an invalid command.
