How to restore media index

I am receiving the following error for all tapes I put in the drive.

dlt7000 tape XXX.026 not in media index when trying to run the scanner command. I recieve a read open error, I/O Error when trying to mount tapes on other drives.

What is causing this and what is the fix?

I have tried mmrecove, nsrck, and scanner I'm getting media index errors for them.
 
scanner is the only command which allows you to re-add deleted file index/save set/volume information.

If the media has been deleted from the index, you can not mount it any more until a scan was sucessful. This is normal.

However, if the scan fails, the question is: has the media been (accidenatlly) be relabeled (by another user/with another program). What does scanner exactly say?
