How do I make the table extend all the way to the top of the page as well as all the way to the left of the page. As you can see there are gaps at the top and left side.

Heres the ticket.
In DW go to Modify>Page Properties (or hit ctrl+j) Set all the margins (top, left, width, and height) to 0.
Or add the following to the body tag:
<body leftmargin="0" topmargin="0" marginwidth="0" marginheight="0">
