Help with forwarding mail to "Smart Host"

I'm running a Windows 2003 SP2/IIS 6.0 server behind a Cisco PIX 506e and have the SMTP service configured to route/forward all email generated by different web sites hosted on that server to a "Smart Host". That "Smart Host" is an SMTP server on my ISPs end. Right now mail isn't getting over there.

Do I need to allow any particular inbound for this to work properly. I'm assuming all outbound traffic will occur by default. I'm just not up to speed on any two-way traffic requirements for SMTP. Sorry if its a dumb question.
 
I know Exchange uses ESMTP. If the SMTP service on IIS uses this as well, then that could be the problem. The 506e uses PIX OS6 that doesn't support ESMTP. If you have this line in the config
fixup protocol smtp 25

Then just put "no" in front of it and give that a try.

Resolved. This is the second issue that I thought was potentially related to the Cisco PIX that has turned out to be something else. It's completely unrelated to the purpose of this thread but the resolution involved configuring the relay settings in IIS properly.

I greatly appreciate everyone's comments to help out with these issues. While the responses I have sometimes received did not immediately resolve my issue they almsot always stimulated the thought process to go in the right direction.
