Help with Form

Hi

I'm new to access and I'm making an airline database and I'm am wondering how you make a form which enables uses to search for a flight. An example is here:


I have a query that pops up and tells you [To:] [From:] and [Date:] But I want it to show up like it does at that site where it is in a form style and not a pop up thing.
 
I'm not quite sure how to edit a thread, but I would just like to say thanks in advance :)
 
Oh and how do you keep the zeros in a row?

I want to keep the time leaving and time arriving in times like 0600 to 0800 but when i type it in, it appears as 800 and 600 with no zeros at the front.

I haven't been on for ages but for the time you need to allow zero lengths because at the moment it automatically deleting zero at the begining often found in the propertises
