HELP! Mail being sent to recipient "Added By System" automatically

An email was addressed from outside our company ONLY to person A at our company. Apparently it also went to person B at our company, because person A saw a reply sent by person B. On that reply, it showed person B (added by system) as being a recipient of the original incoming message, even though the outside sender never copied person B intentionally. I don't think this is a bcc, but might be something set up by the administrator. I can find nothing on this in the help index. Is this part of the "Delegation"? How could this occur?
 
Was person B really listed as the original sender of the reply or did the mail say: 'From person A' and 'Sent by person B'? This could be the case when B has delegation rights on mailbox of A (Access Level: allowed to edit and send mail on behalf of A) which means A has granted B access to his mailfile. Then B can access mail database of A and reply to mail from outside although B has never received the mail directly.
 
1) Check in the Person B's Mailbox the SMTPOriginator Property for the Mail or Check the Sent By in the Mail
2) Check if there any Agents enabled

Thanks for your reply. In answer to your question, person B in fact WAS really listed as the original sender of the reply - - it did NOT say: 'From person A' and 'Sent by person B'. What does that mean?

You can only see the 'sent by' when you send the mail internally to another Notes user within your company. As I understood this mail was sent to an external customer --> 'sent by' is no longer visible and person B (who in your case must have delegation rights to mailbox of user A) gets listed as the sender of the mail ----> for the outside customer it is not visible that B has written the mail in A's database and sent it from there.

Thanks, but let me explain. The INCOMING email was from outside the company to person A, but apparently person B was somehow copied automatically. I understand what delegation rights are (sort of), but the return message which B sent to the outside person is where it showed B as having been "Added by System" to the incoming message. I still don't think that delegation is what happened here, but I don't know what else it could be.
