Help Installing Unity!!!

Hi,
I try to install Unity and it fails out. I see some SQL errors. Here is one I found in the event log. Has anyone seen this?

Cisco Unity Setup encountered an error while running configuration SQL script 'D:\CommServer\Localize\DefaultConfiguration\ENU\ENU_DefaultDatabasePrep.sql' [Error 0x80004005]

A few questions:

1. Is this standard or non-standard hardware (Cisco or non-Cisco approved)?
2. What version of Unity are you trying to install?
3. What operating system are you using (Windows 2000 or Windows 2003 Server)?
4. What Exchange version are you using (Exchange 2000 or Exchange 2003)?
5. Is this a Unified Messaging configuration or Voicemail Only configuration?
6. Is this a new install of Unity or an upgrade?

Before installing Unity did you run CUSPA and CUICA.

I have same problem given below are the details

1. Non-standard hardware non-Cisco approved)?
2. 4.0.1
3. Windows 2000 Advance Server
4. Exchange 2000
5. Voicemail Only configuration
6. New this a new install

Thanks you guys for getting back to me. I got on the phone with Cisco and they finally determined it was a SQL permission problem. I gave the SQL service permissions with a domain admin account and all worked great.
