Ground Start Lines

Has anyone experienced a problem with ground start lines not seizing a line? I have checked the ground as well as polarity on the lines, and they seem to be fine. There is an intermitent problem with dialing an outside line and then having the line drop instead of seizing a line and dialing out.

I have a location where I am having the same problem. We are having problems getting a outside line. Rebooting the phone system seems to clear this up.

 
Thanks for the reply. I actually went through the switch and renumbered each line's outgoing group number to isolate each line when calling out. It turned out to be (1) bad line in the dial-out pool.
 
I am having SBC come out to verify they are setup as Ground Start lines and properly terminated.

We also used the call monitor and noticed that a call was still active that had been disco'd an hour before. The receptionist had tried to do a conference but messed it up and then dialed back out to the remote party. The conference and her new outgoing call remained active after she hung up.

Any ideas on this? We put this system in last fall around October. I don't have the version info in front of me but has there been a service release for these types of problems?
