Ground Start issues

IP 406 R2.1 v15. 16 Port Analog trunk card with 10 main copper ground start lines for use. If I call customer all is fine, they can hold, trans, conf, etc. However, if they initiate the call then no trans, hold, conf etc can be used. I verified all the lines are true ground, polarity is good, pbx has good ground? Any ideas? Also, issues with incoming calls to AA while you listen to the day menu, it automatically sends you to the 1st submenu before you press anything. I have turned of the timeout, but still same issue. I am wondering if the ground start lines are coming in too hot, causing this issues? Any help is as always greatly appreciated?

Does your Telco support answer reversal and do you have an entry in your trunk setup for answer supervision. I have seen a similar fault on Panasonic where the system is waitng for an acknowledgement that the call has been answered before you could transfer the call. When we disabled the system from looking for an answer we could transfer happily.
 
IPO does not allow outbound calls to be transfered to another outbound call.

I have no idea why not but will be reporting it to the 3.0 team as a bug
