getting error message on form after saving to server

Probably a really easy fix. I created a contact form page in page wizard (FP2002). The save file was created in the web. Also I filled in the contact e-mail in the requester box to be notified of requests for information under form properties. However, when I attempt to utilize the form, I get a Frontpage error message.

Found not a fix, but maybe a pointer to the reason that I get an error. I removed the e-mail forwarding, and I no longer get the FRontpage error message. I don't know why. I'm looking into it.
