get a value from combo box

Hi guys,

Can anyone please tell me how can i make a combo box on a dialog box works?
So far I have a dialog box, with a textfield in it, which takes user's input, i.e a customer name, and then create a report for the particular customer name. The problem is, if i mistyped the name, the report won't come out rite. Therefore, i was trying to create a combo box, so i could just choose a name from the list rather than typing the whole thing up. But it just doesn't work.

the report will just come out blank, like there is no data for the particular customer.
