It’s simply a string of menu prompts, in this example 12 of them and the rating is from 1-6.
Answers are stored in a database, including time, date and CLI. Easy to extract using MS Access.

If you need different ratings or number of questions, just alter the menu’s + the wav file that explains the rating options
(The Menu-timeout can either just skip to the next question or have some audio telling them to hurry up)

As it is based on CPxx values, it can only be 16 questions…enough in most cases for a survey.
