Forward on busy issue

When one particular user (Phone A) is busy on an inbound call, the next inbound call to Phone A rolls to Phone B. If both Phone A and Phone B are busy, the second call to Phone A still rolls to the second line on Phone B. Phone A is the only phone in the system that exhibits this behavior. If Phone A is busy with an outbound call or busy with an inbound call on the second line (but the primary line is open), the call rolls to Phone A's voicemail (desired behavior).

All settings ("Fwd no answer", "Fwd on busy", etc.) match all other extensions and the mailbox has been deleted and recreated. Testing with "Fwd on busy" for other extensions produces expected results, but the parameter does not appear to affect this particular extension. Is there somewhere else we should be looking?

I am not positive regarding make/model, but I know our documentation refers to the system as a "Norstar Modular ICS 2.0" and the desk phones are Meridian 7310's.

what are the settings you were checking?
is the phone A the first extension 21, 221?

When you mean inbound/outbound calls - do you refer to internal calls within the system or calls to/from the CO
 
Hopefully this is what you are looking for:

Phone A is extension/DID 2606 (we own 2600-2699).

All call references are external calls.
 
I would remove any fwd on busy settings and just let fwd no answer do the job.

You can also check the prime set for the DID of 2606. If it is set up to phone B, that could cause this behavior.
 
All of the extensions (including Phone A) have "Fwd on busy" set to "None". "Fwd no answer" is set to 2650 (voicemail). Changing "Fwd on busy" for this particular extension doesn't affect the behavior at all, though changing it on other extensions works just fine.

Can you clarify what you mean by "check the prime set"?
 
They mean under lines check what the prime is set to, also under set programing there a place for evey set to change a prime.
