I have an IPO 406v2 @ 4.0.14. I upgraded from 3.2.55 to 4.1.12. After upgrade off site forwarding stopped, all you get is 5 beeps in a row like the line is not available. I downgraded to 4.0.14 this morning thinking it was a bug in 4.1, but the problem followed. I have tried work arounds and even opened a ticked with Avaya. No solutions yet. Any Ideas, I can't be the only one with this problem. Looking at the Monitor, i believe the IPO is inserting the wrong information in the ISDN layers to the network.
 
can you provide a little more detail. Like do u have T1 or Analog trunks???
 
Make sure you don't have any " , " or " . " in the forwarding number. Ours worked fine on our 406 until we upgraded to 412 then we had the same problem.
