Form design received in different format

I've created a custom form for a ticket database. Once after I save and publish it to my 'personal forms', I send it and the recipient doesn't get the form in its original format, it looks like a standard email message. None of the custom buttons and fields are there.

If you are talking about a custom form for Outlook, check if the " Compose page" and "Read page" are the same.
