Extruding Type

Take a look at the graphic at the top of
I did that, based on instructions on a web page that I can't seem to find again, and I need to do it again.

Can you please tell me how I did it?

Using CS2 I enter the type in a document in AI, and I cannot do a 3d/extrude and bevel without first rasterizing the text (they are greyed out), and once I rasterize it, what is extruded is not the type, but rather a rectangular box in which the type sits, i.e.I get a box with the type on the front of the box
