External Call Forward BCM50 Release 2.0

I have a customer with a BCM50 Release 2.0 with a PRI. The customer external call forwards their sets to their cell phones when they are out of the office. Every once in a while the call forwarding stops working and the caller either gets a normal busy signal or a fast busy signal. The only way to restore the forwarding feature is to reboot the BCM50. Any then the forwarding work again for a while

Any ideas how to fix this
 
How many people are doing this?
For every phone forwarded is 2 channels tied up out of your 23....if you have a fractional PRI with only 10 channels or so then you need more.

I suspect the normal busy is it can get a channel out, the fast busy is no channel free to get inbound.

Just my guess.

There are up to 3 people in the office that call forward their sets externally on occasion.
I am aware that for each call forwarded externally 2 channels are used for the duration of the call.
We currently have 10 channels active on the system, and are looking at adding an additonal 6 channels.
The problem is that when I was at the customers site today, I tested the call forwarding and it did not work. I also check the channels and none of them were active. Until I rebooted the BCM50 the forwarding would not work.
 
I was the customer location today, because they were having the same problem. Unfortunately the external call forwarding was working when I arrived. I checked the BCM monitor and all calls were logged.
Ths customer now has 16 channels on their PRI. I was able to make 3 simultaneous calls to a cell phone using external call forward (1 answered, 1 call waiting, and one to voicemail), and the 4th call was busy.

Any ideas to check anything when the problem happens again?
