That's probably a half link, start by replacing the fiber, and, since you are at it, blow some air into the connectors of the fiber daughterboard and fiber receiver cards to clean them up. (make sure is DRY air, and low pressure)
post is right, you may have a bad replacement part, those always kill me. try swapping cards with a working cab. just to make sure it's not a new bad card. the fiber would be my choice.
