If I have a user who sends out an email with an attachment on it to 20 internal email clients does the attachment go out 20 times or can it be setup in a way whereby it's held centrally and every clients pulls it from there.

Exchange uses a process called Single Instance Storage for users on the same server. The email is in one location until a user moves it manually.
 
So all recepients read the attachment from the senders single location?
 
Yes, all users read from the same location in the IS database. Of course, as I said, this has to be on the same server...
