Exchange 5.5 Push Notifications Through PIX

My client has an Exchange server located in the DMZ. The internal mail is working fine with the exception of the automatic push notifications. The user must elicit some activity within Outlook before they are notified of new mail. I did some extensive research that showed a need for an "established" command for the UDP ports, as the push notif. from the server uses indiscriminate UDP port assignments for these notifications. We did this and it still seems to be a problem.
Anyone have any further ideas?
