Excel Chart Sequence

I'm making a bar chart from an excel table that has three columns. The first is name, the second is score in quarter 1 and the third is score in quarter 2. Scores can be from 0-5. The names appear on the chart in reverse sequence, i.e the last row appears first. Also, the score labels, 0-5, show up twice, i.e., 1 1 2 2 3 3 4 4 5 5.
How can I fix this?

Are your series in ROWS or COLUMNS?

What values are displayed on the vertical axis?

Waht values are displayed on the horizontal axis?
