Error: Duplicate data occuring!!

I am using the wizard to create an org chart from an SQL server view. I choose Last Name, Reports To, and First Name.

Everything goes fine, with one small problem.
There are 2 people with the same last names in my query, and I get an error when I try to create the org chart. it says:
"Invalid Data: Chandler. Eliminate Duplicates in the column(field) that uniquely identifies each record"

LastName is NOT my primary key at any time in my database. Visio uses Last Name to build the chart depending on who last name reports to as far as I know.

Visio will ignore this error and build the org chart, but it eliminates one of the duplicate employees. How can I avoid this? I am sure visio can build a chart even if people have the same last name. I mean what if I had 5 Smiths in my plan??

One more Error:

All my position numbers which are 6 digits long only show up as 5 digits when I build using the wizard. If I add choose a record manually the position number shows as the full 6.

Why is it removing the last digit when I build using the wizard?
 
You'd better starting to use a employee or position id/codes in your database, instead of using last name.
However, if you use position id and have 2 people in the same position id/code, visio will gives error message too. Here, we can't use one single position box for 2 different people. I arrange to add sequence number in the second or third position and let the subordinates attached to the first position.
