ERR648 on R4.50W

Dear all,
my system is OPT11c , and software version is Rls 4.50W with voicemail to Rls 13.14.2 , when dial internal vm number(4323),
error code occupied [ERR648 VITN: 00000CCF] ; system not install the ITG card. i already check that's fault of R4.50W , everyone have any patch to solve this bug? thank you.
