@BResource are you using R11.1 FP2 ? I noticed too and the issue is still present in the latest SP3 software, it occours if you try to manage embedded file on a pre-R10 release.
Same system shows it all if you use a R11.1 FP1 Manager, that's why I think it's a Manager Bug which Avaya did not solved yet.
Embedded file management in IP Office Manager requires the service user account used to be in a rights group with Configuration > Manager Operator Rights set to Administrator or Manager.
Alternate route is to use Web Manager, not sure of the rights for that, use a full administrator.
