Echo/quality issues on IP 500 IP phones

We had echo/quality issues on 5621SW IP phones on an IP 500. A crackling deterioration on the near side only on outside calls. After extensive troubleshooting we found the culprit to be the IP 500 VCM 32( We replaced it a number of times they all produced the same result). We overcame by using a legacy card carrier with a legacy 4 port VCM.

Has anyone else experienced a problem using the IP 500 VCM 32?
