On internal or external calls or both. Is the echo near end or far end?
Does the other party hear it as well? If it is only on outside calls, is it specific to a certain geographical area, or always present?
Is it on more than one phone?
Do you have analog lines or digital trunks?
All of these can contribute.
Remember that for echo to be present you have to have some analog component as echo is merely a function of Level and Time (delay).
