Hopefully the Last word on this but as there is little on the net describing anyone actually FIXING this issue I'd like to leave a note for the next sufferer
Problem: DVD drive contents in explorer window populate with first disk inserted but do not update or refresh with new disk insertion. USB Drives on the same system unaffected. Explorer windows continue to show old files when new disk read even as you click down through directories or have used eject command to open drawer.
ThirdParty Videoplayer /music applications can recognise the swap and play the new content even as Explorer is insisting in great detail that the old disk is still there:
PC#1, My problem is very repeatably linked to install of MusicMatch Jukebox 10. Install Jukebox 9 it is fine, update to V10 again in clean standalone install or internet update and the issues come right back.
(cdogg Thanks - I found other references to early versions of nero doing this but I've never used it and it's not on this system which uses MusicMatch or Record Now for burning. Wonder if they share any code?)
PC#2 Similar but less severe probs afecting left hand (folders) window only, were resolved by removing multiple versions of MSN messenger and windows messenger (thanks Kids) and running the Microsoft Autoplay Wizard which found a minor (possibly unconnected) group policies problem in the registry although Autoplay ran before making the registry change.

Conclusion: NO single cause/solution for this issue found here or elsewhere on the web,
Recommendation to others : After the usual DVD-ROM driver updates/reloads and windows system file checks/cleanups
* Try the Microsoft Autoplay wizard (search in google) and * If all else fails then try to reduce the number of installed programmes that might need to hook into the Explorer shell for WHATEVER reason until it goes away then build back to find a culprit /last straw. Suggested Culprits/'last straws' may include multiple MSN messengers, Nero and Musicmatch
I am left wondering if there is actually some limit on the number of external hooks into Explorer before it grinds to a halt as there seems little in common between the programs that have been cited as having pushed systems 'over the edge' and clearly all these programs are working fine in other situations for many other folk.
