DS16 stuck at TFTP Poll Failed 2

I have a 16B module that has issues. I went through the procedure

step by step, when I got to the step where the IPO is off and I power on the 16B module and press the ESC key till I get the Loader message; it never gives me the Loader message, nor does it show anything loading in the TFTP Monitor screen. Here is what the hyperterm screen shows when I turn the unit on before turning on the IPO.

The procedure you posted is correct, I used many many times successfully, I would try to indicate directly the dsaupnv1.bin in the Manager/BootP options as a source file

You can by selecting the file in the folder where you placed the correct bin file, see pic here

How can I stop the boot to erase the flash
As below
*start HyperTerminal - 38400,8,N,1
power up the expansion module and repeatedly press ESC on the PC until you get the Loader message

Don't stop the TFTP Load, just disconnect the Manager from the system, set the BootP as described then re-connect the notebook to the system, you should receive BootP request from the IPO

What version are you running?
There was an issue upgrading to 11.1.2.4

I ran into 4 DG30B modules that bricked after upgrading to 11.1. SP4. I had same issue you did using DTE port to recover them. I was only able to recover 1 out of the 4 DG30B modules by putting the file on the SD card. The other 3 are now resting at bottom of the ocean.

Assuming you didn't just upgrade to 11.1.2.4, I have no help.

If you did upgrade to 11.1.2.4 there was a bug with 30Bs, maybe it also effects 16Bs. They have a hotfix, and/or you could go with 11.1.3 which lists the fix in the notes. But it does specifically mention 30B. I have done the fix on my customer who had several 30Bs with the issue. (So don't throw them in the ocean...
