drivers for rockwell modem

Hello,
I need drivers for ROCKWELL modem,but I can not find
what type of modem is and which drivers I need to install
that modem
Modem work on ISA slot and on modem chip I have
something like
ROCKWELL 96
9752 B50473-7

Pls I need urgent replay

I have a rockwell hcf 56k data fax pci modem on windows 2000 proffesional and it will not allow me to dial up. It results as unable to establish connection or 777 error.
