In the case of laser printers, there is a duplexing tray. The paper goes through once, then is put into the duplexing tray where it is picked up again and goes through the printer again. I've never heard of duplex inkjets but they would likely work the same way.
Yes, I have an HP cp1700d that has the duplex attachment on the back. This just runs the paper through the printer a second time, the other way around.
Yup, smah's got it. My HP 970 cxi it will print the first side--looks like it is going to drop the paper, but it just holds it in the air for a moment or two (to let the ink set). Next, it pulls the paper back in and prints the other side. Mudskipper

Many low to mid-range inkjets offer a two-sided printing feature. When selected, the user is prompted to pick up the output stack, turn it around and reinsert it into the input tray before pressing a key to continue. Saves on paper, but man, they sure puni$h you on tho$e ink cart$!
I know, they should give IJ printers away for what the carts cost !!! I believe reading about the printer division of HP makes more money that all the servers and puters they sell, and I think it was because of the cartridge sales.
It seems the Lexmark is one of the highest, of course, that's what I purchaced a few years ago before I knew anything about anything about computers.
