Domain Search aand registration Program?

Does anyone know where I can find a program to download or copy, that will put a Domain search and registration form on my page? I would like to be able to do some basic configurations change the color add my name etc. I would also like to be able to make up my own price or get a percent for everyone that signs up from my page! Any advice or suggestions about this would be greatly appreciated!

they have an affiliate program it uses their domain name checker on your site. They charge $19yr for domain names but only keep like $11 and send the extra $8 to u. I have been using this service for 2 yrs now and haved 0 problems with good payout.
