Display Field Name as Value

How do I display the field name as a value in my query?

In the query grid:
FieldName: 'feld name'

that does not bring in teh name of the field

The field name is "Home Address" or whatever. SO when I make a query iw ant it to list teh value i select in one column AND in teh second column i want it to display the label of the field name.

 

[PHV]

SELECT [Home Address], 'Home Address' As Label
FROM yourTable
