When I do a format and choose the drive that I believe has failed I get this "selecting c3t5d4 [disk unformatted]
Disk not labeled. Label it now?". Should I just try to relabel it? What else should I look for?

Do you have Raid Manager (rm6) available? You can use that to check your disk.

I want to be good, is that not enough?

I have never used RM6, I will need to check if it is install also. Do you have a specific command in mind?

Thanks.

Yes it seems RM is installed however I don't know which version.

After running the raidutil I've determined the drive has failed therefore I will replace the drive.

Sorry not to have got back to you - been v busy these couple of days. Glad you seem to have resolved the issue - presumably the drive also has an amber light indicating it's failed?
