Disk I/O error? 1

When I try to use a start-up disk to load windows on a hard drive the floppy gives me a error Disk I/O error. What can be causing this problem? Help
 
Couple of tests to do...try a different floppy disk...if same problem check that the floppy drive is installed properly (power cable and 34pin ribbon cable attached the right way)..if still no go try swapping out the FDD as it could be faulty
 
In most cases when i've got this error, the floppy disk had gone bad and i had to use a new one.
 
Make sure that the bios isn't set for a 5 inch floppy. Not usually a problem these days but I still see it on a couple of our Win95 machines when someone has been tinkering where they shouldn't.
