Disable DN 1

I just had a DN disable during the day while in use on an Opt 81c. How could that happen? is it possible to have a dial code to disable a phone and somebody inadvertently dial that code? Any other way it could get disabled?

We recently retired an Option61 that randomly did this maybe twice a month, never the same TN. Things that make you go hmmm. Only once did I trace it to a jack under a desk that would be intermittent when user kicked it; that would disu the TN if phone was in use.
 
I saw the same thing on a 61. When we upgraded it to rel 3 a few years ago, the issue got better but never quite went away. 2-3 phones a month on an 800 phone system, not enough to beat yourself up about. Other 61's with the same release at the time had no issues.
