I know two E/// solutions for your purpouse.
First - device DLR-3. Consist of two parts: suscriber part and station part. Between them is twisted pair (max. distance is 3 km). Max. distance between DBCXXX & subscriber part and between station part & MD110 is 1 km. So, you can have the distance 5 km. between MD110 and digital extention.
Second - Remote Digital Extender 3000.
Give me you e-mail and I send the information about Remote Digital Extender 3000 to you.

how long distance You need?
I have few digital phone connected to MD when distance is more then 3 km.
This line have [red]not any expander [/red] and work fine. (No problems)
Now system is BC 12.1 (SP4) but EQU board are ELU 25 and phone DBC 6xx series.
Cable is not coaxial but twisted pair.
In my opinion You must ( if distance is no longer that 3500 m) try and check initiation extension for this long distance.
