Digital Station 30 Stuck In Boot Cycle

I have a Digital Station 30 (700426216) on my test bench that seems to be stuck in a boot cycle.

I followed this FAQ:
It seems to go through all the steps within the DTE I see however I don't see anything in the TFTP log in Manager.

Multi-Sector Erase
OK
TFTP Poll Sent
TFTP Load Start (Light turns orange)
TFTP Load complete

Then it goes right back to the endless boot cycle, reboots and shows this again and again etc:

I plugged in another DS30 so I know the cable / port etc is good on the IPO.

Anyone have any suggestions or something I may have missed?
 
Tried it on another cabinet (11.1) and got this in the TFTP Log

Check that you have the .bin file in the TFTP directory.
If you are using Manager, that the TFTP Server option is enabled (files, preferences).
