Digit duplication

I have a fxo port attached to a local PSTN. When I call any number of of that port, one digit in the dialed number is randomly duplicated. For example if I dial 937758, the router will dial 9377558 duplicating the 5 digit. Has anyone ever seen that.

What is the connected to the fxo port? Are you dialing from what is connected on the fxo port?

Thanks for the quick response. I have Vonage line attached to one of the fxo ports and local PSTN line to the second FXO port. I dial the Vonage number and get a dial tone from the PSTN and at that time I dial a local number. For example when I dial a local number of 937758, the router randomly duplicates the 5 digt and dials 9377558 instaed of 937758. Here is my outbound dial peer config:

Dial peer voice 2 pots
destination-pattern 3T
port 1/1/1

When I dial the Vonage number the router gives me a disl tone I then dial 3 followed by the local number. In this example I dial the Vonage#, get dial tone and then dial 3 937758 and normally it goes fine but sometimes and it is more often now, the router dials 9377558. Hope that explains it a little better.
