Dialog box for report criteria: date range

Hi All,

I have a database that lists incidents. I need to create a report that allows the user to specify a date range, and a location. I would like to use a dialog box for the selections, but can't figure out how to code it to pass the date range and choice from the combo box. Any help would be appreciated.
