DHCP Wrong Subnet Mask

I have a 2003 DHCP server connected on my core switch.All of my other switches are connected to the core switch as well. The problem I am having is that when a device tries to receive a dhcp address they get a "255.255.255.255" subnet mask. The proper subnet is setup to be "255.255.0.0" we have only one vlan and everyting is on that vlan. I can use any help possible as to why this started happening. somtimes the device get the proper subnet
 
Make sure you don't have something else acting as a DHCP server in your network - such as a wireless router, etc.
 
I ran a scan for Rogue dhcp servers, none where discovered. Could there be routing issue ?
 
There could be a routing issue, but I don't see how that would cause a client to get the wrong subnet mask....

Is there anything in the event logs on the DHCP server that may point you in the right direction? Do you have the Subnet mask set-up in your DHCP configuration?
